Our services usually start with home care provided by a caregiver who is a home health aide or a personal care aide. These services may be prescribed by a physician and often the caregiver works under the supervision of a registered nurse.
These caregivers frequently have a greater knowledge, insight and familiarity of their clients than most anyone else on the client’s health care team. They are with their clients several times a week if not every day. It is through these daily visits and caretaking that caregivers recognize changes in their client’s condition. Identifying these changes and taking action is critical in maintaining the health and well being of our clients.
Whether your loved one is recovering from illness or injury, suffering from a chronic medical condition, or simply needs assistance with routine daily activities, PurposeCare can customize a care plan that supports his or her medical and care needs in the comfort and security of their own home. A care manager serves as our clients’ personal advocate and is responsible for developing holistic care plans.
