Our therapists work with patients at home to rebuild strength, restore function, and regain the independence that surgery or illness temporarily interrupted.
Bathing, dressing, transfers, and mobility support while your loved one’s independence is temporarily limited.
Proper nutrition is essential to healing. Our caregivers ensure your loved one is eating consistently and appropriately throughout recovery.
Post-surgical medication schedules are often complex. Our team helps patients stay on track and flags concerns to the clinical team when needed.
Transportation to and from follow-up appointments, physical therapy, or specialist visits (available in select markets).
For patients with complex recovery needs, a PurposeCare care manager can oversee the full plan to coordinate across service lines and communicate with the entire care team.

A member of our team evaluates the patient’s recovery needs, home environment, medication plan, and follow-up care schedule to build the right support structure.
We develop a recovery care plan in coordination with the patient, their family, and their medical providers.
